We provide comprehensive services for the evaluation and treatment of patients who have been injured on the job.  We are committed to providing the highest quality case so that the injured individual may return to work as soon as medically possible.

Our services include x-rays, bracing, splinting, casting, injections, physiatry, pain management, surgical repair of tendons, ligaments and fractures.  Our rehabilitation staff provides a full range of physical and occupational therapy services, hand therapy and functional capacity evaluation.

Following the initial evaluation and injury treatment, the injured worker is scheduled for regular follow up office visits so that the injury status and treatment program can be updated.

Service Highlights:

• Appointments for initial injury evaluations in most cases can occur the same day with a Physician Extender.  These non-physician practitioners review all cases with the COSM physicians.

• COSM accepts placement on new panels.  COSM accepts only the current Pennsylvania State Workers Compensation fee schedule for payment of services.  We do not participate in discounted provider networks. 

• COSM completes a Workers Compensation Visit Information Form at each visit.  This form is given to the patient.   We will provide a copy of this form to a case manager OR employer if the patient has given proper authorization by signing our release form. (located on this website.).  COSM does not complete specific return-to-work forms submitted by employers.  We place in the patient’s chart any information from employers/case managers for the physician’s review and consideration at the appointment.

• COSM follows the Pennsylvania Workers Compensation Act.  Medical information regarding patient visits will automatically accompany the billing statement when mailed to the insurance carrier.  COSM does not duplicate sending records to other entities without a signed release and prepayment for additional records or reports. 

• Case managers may attend appointments with written consent from the patient.  Our appointment schedulers review our policy with case managers that call to request an appointment.  Our providers only see case managers in the presence of the patient during their appointment.
